Overview This position supports the DCH Care Coordination Department and it's business and clinical processes, with a focus on appropriate utilization of resources, patient transitions across the continuum, appropriate documentation and levels of careThis position is responsible for the management, collection, tracking and trending of data related to Care Coordination housed in several systemsAlso, responsible for designing and delivering data analysis from the identified system and reports created(Midas, Medtech, Tableau, and 3M)Provide support for research and data analysis efforts and assists with special projects for the Health SystemResponsibilities Manages day to day operations of databases that support the collection and integration of data and information used in tracking daily operations and overall organizational performance Acts as a liaison between the external data sources used to compare DCH performance Validates internal and external data quality Transfers data to external data sources as assigned Conducts performance analysis reports including trends, projections, external comparisons and correlations Creates and distributes multiple statistical reports essential to DCH planning and goal achievement DCH Standards:
Maintains performance, patient and employee satisfaction and financial standards as outlined in the performance evaluation Performs compliance requirements as outlined in the Employee Handbook Must adhere to the DCH Behavioral Standards including creating positive relationships with patients/families, coworkers, colleagues and with self Requires use of electronic mail, time and attendance software, learning management software and intranet Must adhere to all DCH Health System policies and procedures All other duties as assigned Qualifications Bachelors' degree in Business, IT, Marketing or related field required; Preferred experience in healthcare data management with research; Microsoft Office intermediate skills required in Word, Excel, PowerPoint and OutlookIntermediate Microsoft Office Access, Excel pivot tables, and Publisher skills preferredKeyboarding experience requiredMust be able to work independently, multitask, organize key projects, and prioritize tasks, with emphasis on accuracy and attention to detailStrong team building skills; Strong oral and written communication skills - electronically, face-to-face and over the phone; Trained focus group moderator preferredMust be able to read, write legibly, speak, and comprehend EnglishMust be able to read, write legibly, speak, and comprehend English.
